Hospice Mesa Medicare

The Interdisciplinary Group/Team, or shortened, IDG/IDT, serves as a collaborative team of hospice specialists to work together and plan for a patient’s care. The IDG is required to meet formally only every 15 days, however the team communicates daily by telephone, secure email, and electronic medical records (EMR) all in real time. The purpose of the formal IDG Meeting is to collaborate with disciplines and resources, utilizing the patient’s plan of care. A Hospice IDG comprises of members from the following disciplines:



Attending Physician



The attending physician plays a crucial role in the proper care of a Hospice patient. The patient’s attending or personal physician assumes responsibility for medical treatments for the patient’s treatment on their involvement in hospice.



Hospice Medical Director



The Hospice Medical Director oversees and provides medical services. This specifically includes: house calls, medication management and as being a medical resource towards the entire team. Even without the, or in collaboration with, the patient’s attending physician, the Medical Director might be responsible for the Attending Physician duties.



Director of Clinical Services



Our Director of Clinical Services behaves as a first point of contact and resource for many clinical staff associated with patient care. The Director accounts for managing and coordinating patient care in addition to regulatory compliance of the hospice.



Nursing Staff (RN & LPN)



The Rn provides direct care by assessing and monitoring the well-being from the patient and is accountable for keeping the IDG informed and updated about the patient’s condition. The hospice nurse can also be a resource for the family or caregivers and is also accountable for teaching regarding looking after the individual. An authorized Practical Nurse provides similar nursing services underneath the supervision of the Rn.



Social Worker (MSW)



Social Workers have in-depth knowledge and expertise in many aspects of our community. These include family and support networks; multidimensional symptom-management; counseling and therapeutic support; grief and loss issues and navigating medical care systems. They may be committed to enhancing a patient and family’s total well being and promoting well-being for all.



Hospice Chaplain



A Chaplain is accessible to supply support to the patient and family within the aspects of religious and spiritual matters. The Chaplain provides non-denominational aid and may be described as a liaison to the patient’s own clergy member. At Hospice Integrity, our Chaplains also can serve as the Bereavement Coordinator, responsible to oversee our bereavement activities and manage counseling support to people experiencing a loss of profits.



Hospice Aide



The Hospice Aide provides patient assistance with bathing, grooming, as well as other personal care activities. They are able to provide companionship and observations for changes in the patient’s condition.
